Tavoitteet

At the end of the course the student is expected to
• Apply scientific knowledge of family nursing in the nursing care, health promotion and family support of children and adolescents with chronic conditions
• Identify common national and international challenges and societal changes in health promotion and chronic disease management with children and adolescents
• Assess the effectiveness of solution-focused and multidisciplinary family support and nursing
• Support and advice families in using services provided by NGOs, church organisations and low-threshold programmes

Opiskelija
- osaa soveltaa tieteellistä tietoa perhehoitotyöstä lapsen/nuoren pitkäaikaissairauden hoidossa, terveyden edistämisessä ja vaikutuksista perheeseen
- osaa nostaa esiin ja tunnistaa yleisiä kansallisia ja kansainvälisiä haasteita ja yhteiskunnan muutoksia lasten ja nuorten terveyden edistämisessä ja pitkäaikaissairauksien hoidossa

Suitable for nurses, public health nurses and midwives to specialize and update their competence, or for a student in a Bachelor’s Degree Program in Health Care who has already completed the relevant basic professional studies (The care of children, youth, and families -studies) in their own degree program. A prerequisite for proceeding to the advanced studies is approx. 120 completed credits or other specific reason which has been discussed and agreed with the study guidance counsellor. For Open UAS students’ mastery of the equivalent knowledge (comparable study module of another UAS or adequate work experience as a nurse).

Soveltuu sairaanhoitajille, terveydenhoitajille ja kätilöille osaamisen syventämiseen ja päivittämiseen tai terveysalan amk-opiskelijalle, joka on suorittanut jo omassa tutkinnossaan aiheeseen liittyvät ammatilliset perusopinnot (lasten, nuorten ja perheiden hoitotyön opinnot). • Plan, deliver and evaluate evidence-based clinical nursing with children and adolescents according to ethical and economic principles
• Discuss challenges and potential health and wellbeing risks relating to health promotion with children and families
• Work in various family nursing settings in compliance with ethical and professional standards, the values of family nursing, children’s rights, privacy legislation and applicable regulations
• Develop professional competencies in dealing with families from different and individual backgrounds and in different, individual life stages and settings
• Anticipate, identify and analyse factors that can promote families’ inclusion or exclusion, and support and guide families in using multiple services
• Independently administer prescribed medical care under supervision
• Is able to use technological applications and is able to analyze ethical principles related to using technology in care of clients / patients
• Perform professional nursing roles in multi-sectoral and multidisciplinary practice

Opiskelija

Opiskelijan tulee

- luoda/päivittää CV Jobiilissa ja etsiä aktiivisesti harjoittelupaikkaa Jobiilista

- sopia arviointiajankohta yhdessä harjoittelun ohjaajan ja ohjaavan opettajan kanssa
- osallistua reflektioihin


Kotihoidon harjoittelu:

Opiskelija;

- osaa huomioida vanhusten moninaisuuden, monikulttuurisuuden sekä elämänhistorian merkityksen hoidossa ja ohjauksessa
-Tuntee kotihoidossa ja avohoidossa toimivan sairaanhoitajan laajan työnkuvan.
-Ymmärtää potilaslähtöisen hoitotyön periaatteet sekä hoitotyön kirjaamisen merkityksen.
-Ymmärtää ja hahmottaa moninaisen kotihoidon kentän.
-Ymmärtää kotihoidon kentän monet yhteistyötahot ja näiden roolin laadukkaan kotihoidon toteutumisen näkökulmasta.
-Osallistuu laajasti kotihoidon eri toimintoihin, sisältäen sekä työn suunnittelun moniammatillisessa tiimissä, että käytännön hoitotyön toteuttamisen kotihoidon kentällä.
-Osaa toteuttaa näyttöön perustuvaa moninaista hoitotyötä kotihoidossa.
-osaa huomioida kotihoidon asiakkaiden moninaisuuden, monikulttuurisuuden sekä elämänhistorian merkityksen hoidossa ja ohjauksessa.
-Suunnitella ja toteuttaa kotihoidon asiakaskäynti ottaen huomioon potilaan ravitsemukselliset, hoidolliset (hygienia, liikkuminen, henkinen hyvinvointi) ja lääketieteelliset hoidon tarpeet sekä arvioida toteutunutta hoitoa potilaslähtöisesti kotihoidossa.
-Toteuttaa kotihoidon asiakaskäynnin itsenäisesti.


Valinnaisen harjoittelun tavoitteet

- Toteuttaa hoitotyötä huomioiden hoitotyön laajan teoriapohjan (sisätautien hoitotyö, kirurginen hoitotyö, kotisairaanhoito ja avohoito, naistentautien-, perheiden- ja lastenhoitotyö).
- Hyödyntää opittua laajaa hoitotyön teoriapohjaa hoitotyön prosesseissa.
- Osaa verkostoitua ja ohjata potilasta ja hänen läheisiään käyttämään tarkoituksenmukaisia sosiaali- ja terveydenhuollon sekä 3. sektorin palveluita.
- Ymmärtää terveyden edistämisen merkityksen sekä yksilön mahdollisuudet ja rajoitukset terveyden edistämisen toteutumisen näkökulmasta.
- Osaa toimia osana moniammatillista työyhteisöä yhteistyössä potilaan ja terveydenhuollon toimijoiden kanssa.
- Ymmärtää avohoidon roolin osana terveydenhuoltoa.
- Hallitsee keskeiset hoitotyön taidot ja toiminnot.
- osaa tunnistaa ja arvioida potilaan ja asiakkaan hoitotyön tarpeita sekä toimia potilaslähtöisesti työssään.
- Osaa hahmottaa keskeiset hoitotyön ongelmat ja huomioida potilaan tarpeet kokonaisvaltaisesti.
- Osaa toteuttaa turvallista lääkehoitoa huomioiden aseptiikka, potilaan ohjaus, turvallisuus sekä teoreettinen tieto.
- Osaa dokumentoida hoitotyötä yksikön ohjeiden mukaisesti.


Eettinen osaaminen

- Osaa huomioida oman osaamisentasonsa (teoreettinen tieto ja kliiniset hoitotyön taidot) ja osaa tarvittaessa pyytää apua turvallisen hoitotyön toteutumiseksi.
- Osaa hoitotyötä ohjaavat eettiset periaatteet ja noudattaa niitä omassa työskentelyssään.
- Ymmärtää mitä salassapitovelvollisuus tarkoittaa. Toimii noudattaen salassapitovelvollisuutta.

• Can apply previous knowledge in health care and researches when encountering clients from different cultural background and ages in a home care environment.
• Be familiar with the administrative work within home care and outpatient care.
• Be familiar with patient centered care and documentations.

At the end of the course the student is expected to
• Be familiar with home care as a working field
• Know who the most relevant collaborators are within home care and knows which occupational groups are part of the home care team/
• Participate in home care staff meetings and can see the connection between the administrative work and the practical field work
• Practice skills that are of relevance for the nurse/public health nurse in home care/outpatient care
• Be able to vary his/her way of work depending on the client's age and cultural background
• Plan, carry out and evaluate a home care visit, prioritize the need of care, estimate and document the need of nutrition, hygiene, mobility and need for pharmaceuticals
• Be able to carry out a home visit independently

LEARNING OUTCOMES FOR OUTPATIENT CARE
Theoretical competence:
• Integrate knowledge from different sciences/courses such as anatomy, physiology, pharmacology, microbiology, pathology, internal medicine, surgery as well as nursing sciences and apply this knowledge in open care units.
• Apply the knowledge to form a comprehensive picture of the care process in open care.
• Understand what resources are available to health professionals when caring for patients with need for care in open care. Understands the importance of health promotion in open care and clients individual possibilities and obstacles to maintain his/hers own health
• Understands the importance of communication during care both in multiprofessional team and between client and caregiver.
• Understands the difference between hospital and open care and is aware the means how to co-operate between organizations.
Competence to act in practical situations:
• Possess basic nursing care skills needed in open care.
• Apply the treatment process, for example, to observe/assess, analyze, plan and implement needed care in open care.
• Meet the patient's underlying problems, needs, and desires, and increasingly see the patient's whole care picture.
• Be able to administer medications used in open care and can give guidance in medical use to clients.

PRA4 consists of:

10 etcs = 267 hours, 8h can be reduced (for CV, placement search via Jobiili, reflections), so total amount of hours are 259 hours.

It means approximately 38 hours per week.

- approved personal aims send by the end on first practice week

- Assessment discussion will be held with the student, the supervising nurse (harjoittelua ohjaava sairaanhoitaja) and supervising teacher in the middle or the end of the practice.
If you have two different practice placements, assessment discussion will be held on both of the placements

• Know who the most relevant collaborators are within home care and knows which occupational groups are part of the home care team/
• Participate in home care staff meetings and can see the connection between the administrative work and the practical field work
• Practice skills that are of relevance for the nurse/public health nurse in home care/outpatient care
• Be able to vary his/her way of work depending on the client's age and cultural background
• Plan, carry out and evaluate a home care visit, prioritize the need of care, estimate and document the need of nutrition, hygiene, mobility and need for pharmaceuticals

Theoretical competence:
• Integrate knowledge from different sciences/courses such as anatomy, physiology, pharmacology, microbiology, pathology, internal medicine, surgery as well as nursing sciences and apply this knowledge in open care units.

• Understand what resources are available to health professionals when caring for patients with need for care in open care. Understands the importance of health promotion in open care and clients individual possibilities and obstacles to maintain his/hers own health

Valinnaisen harjoittelun tavoitteet

- Toteuttaa hoitotyötä huomioiden hoitotyön laajan teoriapohjan (sisätautien hoitotyö, kirurginen hoitotyö, kotisairaanhoito ja avohoito, naistentautien-, perheiden- ja lastenhoitotyö).
- Hyödyntää opittua laajaa hoitotyön teoriapohjaa hoitotyön prosesseissa.
- Osaa verkostoitua ja ohjata potilasta ja hänen läheisiään käyttämään tarkoituksenmukaisia sosiaali- ja terveydenhuollon sekä 3. sektorin palveluita.
- Ymmärtää terveyden edistämisen merkityksen sekä yksilön mahdollisuudet ja rajoitukset terveyden edistämisen toteutumisen näkökulmasta.

Suitable for nurses, public health nurses and midwives to specialize and update their competence, or for a student in a Bachelor’s Degree Program in Health Care who has already completed the relevant basic professional studies (The care of children, youth, and families -studies) in their own degree program. A prerequisite for proceeding to the advanced studies is approx. 120 completed credits or other specific reason which has been discussed and agreed with the study guidance counsellor. For Open UAS students’ mastery of the equivalent knowledge (comparable study module of another UAS or adequate work experience as a nurse).

Soveltuu sairaanhoitajille, terveydenhoitajille ja kätilöille osaamisen syventämiseen ja päivittämiseen tai terveysalan amk-opiskelijalle, joka on suorittanut jo omassa tutkinnossaan aiheeseen liittyvät ammatilliset perusopinnot (lasten, nuorten ja perheiden hoitotyön opinnot). Suitable for nurses, public health nurses and midwives to specialize and update their competence, or for a student in a Bachelor’s Degree Program in Health Care who has already completed the relevant basic professional studies (The care of children, youth, and families -studies) in their own degree program. A prerequisite for proceeding to the advanced studies is approx. 120 completed credits or other specific reason which has been discussed and agreed with the study guidance counsellor. For Open UAS students’ mastery of the equivalent knowledge (comparable study module of another UAS or adequate work experience as a nurse).

The internship can be completed in a variety of child, adolescent and family care settings where nurses work.

15 ECTS = 400,5 hours.

The number of training hours includes the orientation / planning, implementation and evaluation of the training in different ways

Suitable for nurses, public health nurses and midwives to specialize and update their competence, or for a student in a Bachelor’s Degree Program in Health Care who has already completed the relevant basic professional studies (The care of children, youth, and families -studies) in their own degree program. A prerequisite for proceeding to the advanced studies is approx. 120 completed credits or other specific reason which has been discussed and agreed with the study guidance counsellor. For Open UAS students’ mastery of the equivalent knowledge (comparable study module of another UAS or adequate work experience as a nurse).

Soveltuu sairaanhoitajille, terveydenhoitajille ja kätilöille osaamisen syventämiseen ja päivittämiseen tai terveysalan amk-opiskelijalle, joka on suorittanut jo omassa tutkinnossaan aiheeseen liittyvät ammatilliset perusopinnot (lasten, nuorten ja perheiden hoitotyön opinnot). Edellytyksenä syventäviin opintoihin siirtymiselle on n. 120 opintopistettä tai opon kanssa sovittu jokin muu erityinen syy. Avoimen amk:n opiskelijalla vastaavien tietojen hallinta (muiden AMKien vastaava opintojakso tai riittävä työkokemus sairaanhoitajana).

Opintojakson nimi suomeksi: Lasten, nuorten ja perheiden kliinisen hoitotyön syventävä harjoittelu.
AHOT 5/2021: Voidaan ahotoida vain, jos opiskelijalla on aikaisempi sairaanhoitajan tutkinto tai vastaavilla sairaanhoitajan tutkintoon johtavilla korkeakouluopinnoilla. 

Diakin opetussuunnitelmaa noudattavat suomenkieliset ryhmät tekevät tämän harjoittelun 15 opintopisteen laajuisena. Nursing-ryhmät tekevät tämän harjoittelun opetussuunnitelman mukaisesti 20 opintopisteen laajuisena.

• Understand the documentation and the importance of continuity of patient care
• Use different health care recommendations and manuals in support of their activities and learning
• Develop their ethics and can identify, analyze and reflect on ethical problems
• Have basic skills to relates to the patient’s basic care area, such as the patient personal hygiene, nutritional supply or condition and observation of the patients vital life functions
• Understand the importance of patient and drug safety
Internal medicine
• Understand the role of the nurse in multiprofessional cooperation in the care of internal medical patient
• Understand the meaning of drug treatment for patients with internal medical diseases
• Know how to care for parenteral patients fluid and drug treatment
• Understand the inpatient care nature and form an overall picture of care process
• Can according to the patient’s health and symptoms choose and perform appropriate care activities
• Understand the importance of patient guidance and practice to handle it in different situations
Surgical care
• Understand the role of the nurse in the professional cooperation in care of surgical patients
• Can implement the surgical patient’s basic care
• Know the main drug groups and their indications/contractions in the care of surgical patients
• Can judge the condition of the patient, both clinically and with the help of observations instruments
• Understand the actions that are being taken at the ward, be able to prepare for the interventions as well as assist with these
• Understand the care and prevention of pain and nausea with medical and non-medical methods
• Can guide and inform the patient before, during and after surgery

Competency aims:
The aim of the course is the student
• Can apply previous knowledge in health care and researches when encountering clients from different cultural background and ages in a home care environment.
• Be familiar with the administrative work within home care and outpatient care.
• Be familiar with patient centered care and documentations.

Learning outcomes:
At the end of the course the student is expected to
• Be familiar with home care as a working field
• Know who the most relevant collaborators are within home care and knows which occupational groups are part of the home care team/
• Participate in home care staff meetings and can see the connection between the administrative work and the practical field work
• Practice skills that are of relevance for the nurse/public health nurse in home care/outpatient care
• Be able to vary his/her way of work depending on the client's age and cultural background
• Plan, carry out and evaluate a home care visit, prioritize the need of care, estimate and document the need of nutrition, hygiene, mobility and need for pharmaceuticals
• Be able to carry out a home visit independently

LEARNING OUTCOMES FOR OUTPATIENT CARE
Theoretical competence:
• Integrate knowledge from different sciences/courses such as anatomy, physiology, pharmacology, microbiology, pathology, internal medicine, surgery as well as nursing sciences and apply this knowledge in open care units.
• Apply the knowledge to form a comprehensive picture of the care process in open care.
• Understand what resources are available to health professionals when caring for patients with need for care in open care. Understands the importance of health promotion in open care and clients individual possibilities and obstacles to maintain his/hers own health
• Understands the importance of communication during care both in multiprofessional team and between client and caregiver.
• Understands the difference between hospital and open care and is aware the means how to co-operate between organizations.
Competence to act in practical situations:
• Possess basic nursing care skills needed in open care.
• Apply the treatment process, for example, to observe/assess, analyze, plan and implement needed care in open care.
• Meet the patient's underlying problems, needs, and desires, and increasingly see the patient's whole care picture.
• Be able to administer medications used in open care and can give guidance in medical use to clients.
• Can do documentation of care/ clients condition on the way it is done in the unit.
Ethical competence:
• Be aware of their own level of competence and seek guidance when knowledge and skills are lacking.
• Follow the ethical guidelines for care.
• Understand the importance of confidentiality.
• Understand the importance of ethics in cooperation between staff and client and family members.
